For the first time in 2005, Lipscomb introduced an incentive program for those employees who are covered under Lipscomb's insurance plan. The program is designed to encourage our medical insurance participants to life healthier lifestyles by providing a financial incentive in the form of a one-time $200 medical insurance premium reduction. We are delighted to offer this program again this year. Each school year, the program begins in August and ends the following May.

To be eligible for the incentive you must be a full-time employee of Lipscomb and a participant on our medical insurance plan. If a husband and wife both meet these requirements they will both be eligible for the incentive. The requirements that must be met to earn the incentive are:

  • Complete a Health Risk Appraisal - The completion of this form serves as the entry point into the program. If you completed a health risk approsal (Personal Wellness Profile) in 2005 or 2006, you do NOT need to fill out another one.
    • If you are new to the incentive program, call Ruth Henry at extension 5757 and request a copy of the questionnaire. Complete the form and send it to Ruth Henry, Department of Kinesiology, by September 14, 2007. You may NOT accrue points until the HRA has been turned in.
    • For those who participated in the incentive program in 2005-06, you will automatically be re-enrolled.
  • Accumulate a minimum of 50 wellness points by participating in a combination of the activities below. Specific opportunities below are described in detail under wellness points and activities .
    • Regular Exercise - 50 points (5 points per month) Email "Monthly Exercise Report" form from website to Matt Tiller at end of each month.
    • Exercise Contest - 20 points/contests. We will have 2 each school year
    • Exercise Events - 5-10 points/event
    • Wellness Seminars - 10 points/seminar
    • Wellness Consultation - 10 points. Only your initial consultation will earn points.
    • Fitness Assessment - 10 points
    • Personal Trainer - 10 points total for 4 sessions (available spring semester only)
  • Ruth Henry/Matt Tiller will record wellness points on a spreadsheet as they are earned (S:\documentation\wellness incentive). You may view your wellness points on this file at any time. Participants will be listed under your L-numbers for privacy--no names are included on the spreadsheet.
